Before Your First Visit:

1. Complete your Admission Packet before your scheduled appointment. 

 

2. What to bring with you: 

  •  Photo ID (patient or parent/guardian)
  • Insurance Card(s) 
  • Doctor’s Prescription for physical therapy
  • PT Admission Paperwork (you may fill this out at the clinic prior to your evaluation)

 

3. Wear clothing that will allow access to the area that will be treated.

What to Expect During Your First Visit:

  1. The physical therapist will begin by reviewing your past medical history and asking questions about your current condition.
  2. The physical therapist will then perform a detailed examination of the area(s) to be treated.  This may include joint measurements, inspection, strength, flexibility, range of motion, swelling, posture, balance, stability, palpation, gait observations and/or other procedures depending  your specific diagnosis, impairments or limitations.
  3. The physical therapist will then evaluate the results of the objective examination and make an assessment of your physical impairments and/or functional limitations.
  4. Your therapist will discuss your personal goals and prior level of function.  Your desired functional level and specific activities will assist the therapist in working with you to develop short and long term goals, a treatment plan tailored specifically for your needs and a plan of care that will guide your treatment.
  5. The therapist will discuss your diagnosis and prognosis with you.
  6. In some cases treatment will begin on the first visit after your initial evaluation has been completed.
